Individual Clinical Facilitation

Client’s individual case assessment and coordination are crucial components of any chronic pain management program and the development of a patient-centered pain management plan. 

1. Comprehensive Pain Assessments: The first step in managing your chronic pain is to conduct a comprehensive pain assessment. This includes understanding the patient’s history, current treatment, medications and conducting physical and psychosocial assessment to identify personally relevant goals, and preferences. Client’s will have an individual sessions conducted by a highly experienced clinical facilitator / Pain coach for a period of 12 months to provide guidance to manage their pain. 

2. Development of a Patient-Centered “Pain Management Plan”: Based on the comprehensive pain assessment, a patient-centered pain management plan is developed. This plan should be tailored to the individual’s needs and preferences, and should include strategies for managing pain, improving function, and enhancing quality of life. This plan will generally involve attending the group education sessions along with any individual allied health appointments that have been identified as potentially beneficial to the presenting condition.

3. Coordination of Care: Effective pain management plan requires coordination among various healthcare providers. This includes the patient’s primary care GP, pain specialists, and other Allied healthcare professionals involved in the patient’s care. Regular communication and collaboration among these providers helps ensure that the pain management plan is effectively implemented and adjusted as needed.

4. Regular Monitoring and Adjustment of the Pain Plan: The pain management plan should be regularly reviewed and adjusted based on the patient’s progress and feedback. This allows for the plan to be fine-tuned to better meet the patient’s needs over time.

After attending number if these sessions client will be booked again to see the clinical facilitator for follow up sessions. These appointments generally take place in 3, 6, 12 months. The follow up sessions designed to assess the effectiveness of the any treatment implemented from the initial appointment and make changes, as necessary. The last appointment will be the 12 month follow up, where we develop a discharge plan with the help of the the client for ongoing self-management once they completed the program.

5. Integration with Other Services: Chronic pain management should be integrated with other services, such as mental health services and social support services. This can help address the complex needs of individuals with chronic pain.

Persistent pain is most effectively addressed through a comprehensive multidisciplinary behavioural management method known as the biopsychosocial approach. This integrates with doctors medical, physical, and psychological treatments.
