Information requests
Your name, email and/or phone number is recorded when you request to contact Arche Health or subscribe to the online bulletin. Your information will only be used for the purpose that you have provided it for and will not be added to a mailing list or disclosed for any other purposes without your consent.
Completing referral forms to participate in Arche Health’s programs
There are programs implemented by Arche Health that allows for the client to self-refer. This is through completing an online referral form that will require your personal details including details of the health concern that led to you requesting to be part of the program. These forms are either faxed or emailed to the relevant staff member at Arche Health.
Arche Health treats this information with the strictest of confidentiality and your details will only be accessed by staff members and health professionals who are directly involved in the provision of your care.
The following privacy measures are implemented for the storage of your information:
- In paper form – received and stored in a secure lockable location
- Electronic form – password and firewall protected
- Will not be taken from Arche Health unless for a specified purpose and with your consent
- We will destroyed or permanently de-identified when no longer required by Arche Health
As Arche Health is a government funded organisation, the information you provide may be utilised for reporting purposes. In such instances your information will be de-identified and be reported on as part of a larger group of clients.
